The human embodiment of the Island that is "Fantasy Island."
She chose Ruby to be the 'new tattoo' bearer of the island, and also sometimes takes human form to be with her. However Isla's existence is confusing at best, and she may not be real. Or at least, she's as real as anything else on Fantasy Island.
